WMMRC - Request MRC or Zipcode for printable CORI form

Thank you for your interest in volunteering. Please use one of the choices on this page to create a CORI form, downloadable in PDF format.

All volunteers are affiliated with a local MRC. Your CORI Form must be sent to an MRC unit for verification.

You can choose the MRC with which you are affiliated by selecting one from the drop down menu and clicking submit to create your printable CORI form.

Alternatively, you may be assigned to an MRC based on your 5-digit ZIP Code. If you prefer this method, please enter your 5 digit ZIP Code in the text area and press submit to let us assign you to your local MRC.
